OHSAA Pre-Participation Physical Exam Forms
In order to participate/try out for a sport at SUA, all student-athletes must have a valid OHSAA sports physical on file. The completed physical must be submitted to the Athletics Office or uploaded to your daughter's FinalForms account before the first day of tryouts/practice.
Please note: Students who do not have a valid physical on file prior to tryouts will not be permitted to participate.
Once the forms have been completed, signed, and dated by a physician, you may:
- Upload the completed physical to your daughter's FinalForms account, or
- Turn in a hard copy to the Athletics/Athletic Trainer Office
