Please complete the first 2 sections of the packet carefully, completely and thoughtfully. Please ask your parents to do the same for section 3 (parents may complete this section together or separately). This process is extremely important for several reasons:

• The process helps you think about yourself - your past, present and future; your strengths and weaknesses; your goals and aspirations – as you begin your college search.
• Hopefully, the packet will encourage further communication with your parents about college and your future.
• Teachers and counselors will use this information to write your college letter of recommendation. The more information that is shared, the better prepared these professionals will be to write letters that most reflect you as a whole person.

2. Respond to all questions
3. Print one copy and submit to Guidance (order by page number and staple).
4. You will need to print a second copy for the teacher writing your letter of recommendation. You must print this from your disk, the guidance department will not be making copies. (Some teachers will ask you to complete a personalized sheet as well.)
5. Put your disk in a safe place (preferably with the rest of your college search materials). You may need to print future copies of the packet. You may chose to print a copy for yourself or save the packet to your hard drive in case your disk becomes damaged
