Class of 2017 Admission Forms

Can be completed on the computer then printed or printed and filled in by hand:

Application and Entrance Test Registration Form (Part of the Application Form OR can be done at another school) Test fee is $30 and must accompany the registration from.

Release of Records Form (Provides current contact information and allows your daughter’s current school to release her high school record form and appropriate information for consideration by the Admissions Committee)
 

Transfer Application (Provides current contact information for a student wishing to transfer to St. Ursula Academy and allows the student to make a brief personal statement.)


Testing Accommodation Request and Release Form (For students with a documented learning disability only) All forms are due by Nov. 2nd.

Financial Aid Application (This form will become available in October and needs to be sent to PSAS by November 19th. This information is then used to award financial aid packages from St. Ursula.)
