2009- 2010 Admission Forms
Can be completed on the computer then printed or printed and filled in
by hand:
Release of Records Form
(Allows your daughter’s current school to release her high school record
form and appropriate information for consideration by the Admissions
Committee)
Entrance Test
Registration Form (Part of the Application Form OR can be done at
another school) Test fee is $25 and must accompany the registration
from.
Application Form
(Provides current contact information and allows the student to make a
brief personal statement.)
Transfer Application 2010-2011 (Provides current contact information for a student wishing to transfer to St. Ursula Academy and allows the student to make a brief personal statement.)
Testing Accommodation Request and
Release Form (For students with a documented learning
disability only) All forms are due by Nov. 6th.
Financial Aid Application (This form will be available to download in
October and will be sent to and processed by PSAS. This information is
then used to award financial aid packages from St. Ursula) All forms
must be postmarked by Dec. 4th.
