Testing Accommodations
Saint Ursula Academy allows students who have disabilities the opportunity to take the High School Placement Test with accommodations. The accommodations offered for this test include a reader, extended time, and an enlarged print format.

In order to be eligible, a student must have been identified as having a disability and must have a formal written plan (IEP, ISP, 504 Plan or Accommodation Plan) at her grade school that describes her accommodations.  In addition, the accommodations must be used on a regular basis on classroom tests as well as on group administered, standardized tests such as the Terra Novas.

A student wishing to take the HSPT with accommodations must forward the following two items by November 5th, 2010:
* a copy of her current Individualized Education Plan, Individualized Service Plan, or Accommodation Plan
* a signed copy of the Authorization for Release of Accommodation Information form. The Authorization for Release of Information form gives St. Ursula Academy permission to release your daughter’s accommodation information to the other schools that are receiving her High School Placement Tests scores (Link to authorization form.)

REMINDER
Students applying to Saint Ursula Academy’s Educational Services Program for Students with Learning Disabilities or ADD must forward all pertinent application materials to Ms. Geri Walsh, Educational Services Program Coordinator, Saint Ursula Academy, 1339 E.
