Facilities Request Form

Complete form and click on the submit button at the end of the page.  Print out the confirmation page that comes up next for your records.  E-mail address and Full Name are required.

Note: To check if an event has been scheduled, go to Outlook and then Public Folders (left column at the bottom.)  Click on All Public Folders, then Facilities Request, then Main SUA Calendar.  This is where all events are scheduled once approved by the Facilities Department.  If your event is scheduled here you do not need to complete the Request Form unless there is a change or revision.

Portable Mic is needed (yes or no)       
The portable microphones are the little black boxes that look like a carry-on suitcase. The sound comes from the little black box.  This is used for large groups, such as individual classes or clubs with a lot of members.  The only set up necessary is for it to be plugged into electric anywhere in the gym, theatre or dining room.  If one is used, the large sound system with the mixer board is not necessary. 
Sound system for Gym is needed (yes or no)      
This is the sound system that is usually used for mass and all school assemblies in the gym where all bleachers are out.  The sound comes from the speakers in the ceiling of the gym.  It is the big black box  with a mixer board  that uses wireless mics for the podium or for a speaker to wear.  There are also 5 microphones that can be hard wired to it. It requires a person to man the mixer board and a set up of many wires to the sound system network and the mixer board.  If this is used, the portable mic is not needed.
